Send questionnaires and follow responses
Questionnaires collect structured feedback from players and staff. The Parameter Catalogue defines each question, the report template defines the questionnaire, and the sendout defines when and to whom it is sent.
What you will achieve:
Prepare a recipient-friendly Questionnaire template
Choose a one-time, recurring, or event-based schedule
Monitor delivery, response status, and result locations

1. Prepare the questions
In Report builder > Parameter catalogue, create a parameter with the Questionnaire question type or import a supported system parameter.
Keep the parameter name short because it is used in tables, charts, and overviews. Write the complete question in the recipient-facing question text.
Example:
Parameter name: Fatigue
Question text: How fatigued do you currently feel?

2. Prepare the template
Create or open a report template configured for Questionnaire. Select the appropriate category, add the questions, organize the order, save it, and keep the template active.

3. Create the sendout
Open Questionnaires.
Click Create sendout.
Select the active Questionnaire template.
Choose the schedule:
One-time for one defined collection;
Recurring for repeated check-ins; or
Event-based for questionnaires connected to sessions or gamedays.
Configure the send time, response window, and reminder.
Select the audience of players or staff.
Review the Summary carefully.
Create the sendout.
Important: Creating an immediate sendout may notify recipients by app or email. Use a future schedule or test audience while validating a new workflow.

4. Recipients receive the questionnaires
Recipients can receive the questionnaire in the app or through an email link. The email link can be completed without logging in or having a registered planet.training account, provided the recipient has a valid delivery address.

5. Monitor the sendout
Open the sendout to review its dashboard. Monitor recipient status, completed and missing responses, answers, response rate, reminders, and the configured schedule.
Use reminders carefully so recipients are informed without receiving unnecessary messages.

6. Find the results
Results appear in the sendout dashboard. Each player and staff profile also contains a Questionnaires tab with individual assignments, statuses, and responses. Supported Load Management and Wellness questionnaires additionally feed Player Monitoring.
How this connects
Report Builder supplies the questions and template. The sendout supplies timing and audience. Profiles show individual history, while supported system metrics activate additional Player Monitoring calculations and charts.
You are finished when:
The template is active and selectable.
The schedule and audience match the intended use.
You know where to review response status and individual answers.
